  🚨 TO TRY OUT, YOU MUST:
  1️⃣ Use the QR code provided to fill out and print the 2024-2025 Athletic Consent Form (4 pages).
  2️⃣ Submit the completed forms to Mr. Matthew Perez’s file organizer (outside his office near Ms. Ajaka’s).
