Forms

By the end of each semester, you must have completed 20 service hours for the outside community
Large green and small yellow documents are required for weekly commitments. 
Service hours will begin to be documented once the BLUE sheet has been handed into the Service Office. 
Orange documents are meant for SINGLE events. 
Small green Documents are to document service to the school.  

Every year, even for the same organization, Committed service begins earning credit after the green / yellow forms are submitted.
